SPONSORSHIP ASSISTANCE:  Complete the form below if you DON'T HAVE a quote  from a 3rd party business/vendor for the item you want to be sponsored.  We will assist in getting you a quote. 

If you DO HAVE a past invoice or new quote you do not have to complete the form below...

There are NO LIMITS to how many items you can submit for Sponsorship Dollars (Submit all you can).

There are NO LIMITS to how often you can request or receive Sponsorship Dollars (Submit as often as needed). 

The combined dollar amount of your items or online service must be more than a $500 Value.

After submitting your form you will be notified by phone or email to confirm your request for assistance within 24  hours.
